Please Note as of 7/11/2024 we are accepting new patients but however due to the overwhelming request daily we receive,  we may not respond to your request promptly.  Also due to our schedule we may not get you an appointment right away or in the near future and could take a few weeks up to a month before we can get you in.  Please take this into account when requesting to be a new patient.  Thank you.

To be completed by anyone wanting to become a new patient of Dr. Sanchez or FNP Mabel Cruz.  Please do not book an appointment until we have you as a patient.  In the following fields you will have the opportunity to request a date for your first appointment.  Once we have you established, you will be contacted with an appointment.  If you are already a patient, please do not complete this page and click the “Portal for current Patients” from the main menu.

    •  MEDICAID, HUMANA MEDICAID, SUNSHINE.  
    •  WORKER COMP CLAIMS NOR DO WE DO AUTOMOBILE INJURY CLAIMS. 
    • NO HMO PLANS

WE ARE ACCEPTING:

• AARP UHC
• AMBETTER
• AETNA 
• BCBS PPO plans only
•  MEDICARE, TRICARE, WELLCARE,
•  UHC PLANS.
• and we are conducting Immigration physicals (IMMIGRATION PHYSICALS BE ADVISED: before an appointment can be scheduled you MUST have your completed, up-to-date Immunization Record.  You will be required to upload it so it can be verified before appointment scheduled. You will receive an email from Health Fusion to complete your Patient Portal setup.  This is where you can complete required HIPAA documents before your appointment, schedule your first appointment.  In the future, request refills, view results and medical records, medication lists, and send messages to the doctor and staff.
