- The Direct Primary Care payment model is simple. For a flat, recurring monthly fee, patients have access to unlimited primary care services such as routine or urgent office visits, wellness or preventative exams, telemedidine visits, select in-office minor procedures, discounted labs, and more. Pricing is available for individual patients but family plans are also available for better cost-savings. No additional copays are collected!

Family Care Center Corp Care Policy is to charge an enrollment fee per membership upon enrollment which is non-refundable. The monthly membership fees are non refundable. A written notice for termination of service is required. Membership to Family Care Center Corp is a subscripton-based model and membership fees are billed monthly. A method of payment is required for enrollment and must be eligible for auto-draft.
